IMPORTANT REMINDER:
Please ensure that all requirements are complete before going to the office to avoid delays and to make the process faster and more convenient.
Step 1: Secure and Accomplish the Application Form
- Get the application form from the Persons with Disability Affairs Office (PDAO), Malabon City
- Or download it here: https://pwd.doh.gov.ph/downloads/PRPWD-APPLICATION_FORM.pdf
Step 2: Prepare the Required Documents
Bring the following requirements:
- Accomplished Application Form
- Four (4) pieces of 1x1 ID pictures (white background)
- Barangay Certificate of Residency (for PWD purposes) or a valid government ID indicating Malabon as home address
- Certificate of Disability / Medical Certificate (Must indicate diagnosis and classification of disability)
Note: If the disability is apparent or visible (e.g., physical condition), a medical certificate may no longer be required.
Step 3: Submit Your Requirements
Submit all documents to:
Persons with Disability Affairs Office (PDAO)
2nd Floor, Malabon City Hall
Office Hours: Monday to Friday, 8:00 AM – 5:00 PM
