OFFICE: SANITATION
HOW TO GET A HEALTH ID OR HEALTH CERTIFICATE FOR WORK DIVISION
STEP 1: Go to the Sanitation Division Office 2nd Floor, City Hall
- Fill out the Application Form upon arrival.
STEP 2: Submit the Required Documents
For Food Handlers:
- Chest X-ray Result (Original and Photocopy – valid for 6 months)
- Fecalysis / Stool Examination Result (Original and Photocopy – valid for 1 month)
- 1x1 ID Picture
- Valid ID (Photocopy)
For Non-Food Workers:
- Chest X-ray Result
- 1x1 ID Picture
- Valid ID (Photocopy)
STEP 3: Pay the Required Fees
Treasury Department – 3rd Floor
- Health Certificate Fee: ₱50.00
- For Meat Handlers: Additional ₱100.00
STEP 4: Attend the Required Seminar
Attendance in the seminar is mandatory before the Health ID/Certificate can be issued.
Processing Time: Estimated processing time is 30–45 minutes, depending on the volume of applicants.
IMPORTANT REMINDER:
Please ensure that all requirements are complete before going to the office to avoid delays and to make the process faster and more convenient.
