Step 1: Visit the OSCA Office

  • Go to the Office of the Senior Citizens Affairs (OSCA) in your city or municipality.

Step 2: Fill Out the Application Form

  • Request and complete the Senior Citizen ID application form at the office.

Step 3: Submit the Required Documents

Bring the following requirements:

  • One (1) Valid Primary ID (e.g., Passport, NBI Clearance)
  • One (1) Valid Supporting Document

o Birth Certificate or Baptismal Certificate

o Marriage Certificate (for married women, if applicable)

  • One (1) Secondary ID (e.g., Voter’s ID, PhilHealth ID, Barangay ID)
  • Two (2) pieces of 1x1 ID pictures

Step 4: Wait for Processing

  • Submit your documents for verification and wait for further instructions regarding the release of your OSCA ID.

Step 1: Fill Out the Application Form

  • Secure and properly accomplish the required application form.

Step 2: Submit the Required Documents

  • Submit the completed form together with all required supporting documents.

Step 3: Verification and Validation

  • The office will check and validate the submitted requirements.

Step 4: Payroll Preparation and Review

  • The office will prepare and review the payroll for approval.

Step 5: Notification of Release Schedule

  • The claimant will be informed of the schedule for the release of cash assistance.

Step 6: Release of Burial Assistance

  • The burial assistance will be released and distributed to the qualified claimant.

OFFICE: SANITATION

HOW TO GET A HEALTH ID OR HEALTH CERTIFICATE FOR WORK DIVISION

STEP 1: Go to the Sanitation Division Office 2nd Floor, City Hall


  • Fill out the Application Form upon arrival.

STEP 2: Submit the Required Documents


For Food Handlers:

  • Chest X-ray Result (Original and Photocopy – valid for 6 months)
  • Fecalysis / Stool Examination Result (Original and Photocopy – valid for 1 month)
  • 1x1 ID Picture
  • Valid ID (Photocopy)

For Non-Food Workers:

  • Chest X-ray Result
  • 1x1 ID Picture
  • Valid ID (Photocopy)


STEP 3: Pay the Required Fees

Treasury Department – 3rd Floor

  • Health Certificate Fee: ₱50.00
  • For Meat Handlers: Additional ₱100.00

STEP 4: Attend the Required Seminar

Attendance in the seminar is mandatory before the Health ID/Certificate can be issued.

Processing Time: Estimated processing time is 30–45 minutes, depending on the volume of applicants.
