Issuance of Certification of Duly Registered Cooperative

This refers to the issuance of a Certification from the City Cooperative Development Office (CCDO) to cooperatives operating in Malabon City.

The certification serves as proof of the cooperative’s legitimate operation and must be presented to the Business Permits and Licensing Office (BPLO) when applying for a new or renewal of a business permit.

• Barangay Clearance
• Photocopy of Community Tax Certificate
• List of Officers
• List of Members

For Renewal of Business Permit:

• Photocopy of Latest Certificate of Compliance
• Photocopy of Latest Cooperative Annual Progress Report (CAPR)
